What is Kidney Failure / CKD?

Chronic Kidney Disease (CKD) is progressive loss of kidney function, most commonly caused by diabetes, hypertension, glomerulonephritis or polycystic kidney disease. When kidney function falls below 15% (Stage 5 / End-Stage Renal Disease / ESRD), kidney replacement therapy — dialysis or transplant — is required to sustain life.

⚠️ Warning Signs: Swelling in legs/face, frothy urine, blood in urine, reduced urine output, fatigue, nausea, itching, high BP difficult to control. Early evaluation can delay dialysis.

CKD Staging & Management

StageeGFRDescriptionAction
Stage 1–2>60MildControl BP/sugar, nephrology monitoring
Stage 330–60ModerateDiet management, anaemia treatment
Stage 415–30SevereTransplant evaluation, AV fistula creation
Stage 5 (ESRD)<15Kidney FailureDialysis or Kidney Transplant

Urology Role in Kidney Failure Management

  • AV Fistula Creation — surgical creation of arteriovenous fistula for haemodialysis access; ideally 3–6 months before starting dialysis
  • Kidney Transplant — pre-transplant urological evaluation, donor and recipient surgery
  • Obstruction Relief — treatment of ureteric obstruction, stones or BPH contributing to kidney impairment

Frequently Asked Questions

Early CKD is often asymptomatic. Warning signs that appear as kidney function deteriorates: swelling in legs and face (fluid retention), frothy/foamy urine (proteinuria), blood in urine, reduced or absent urine output, fatigue, nausea, itching, loss of appetite, and hypertension difficult to control. Any of these warrant immediate renal function testing (creatinine, eGFR, urine albumin).
An AV fistula (arteriovenous fistula) should ideally be created 3–6 months before the anticipated start of haemodialysis — when eGFR falls to 15–20 mL/min (Stage 4–5 CKD). This allows time for the fistula to 'mature' (enlarge) before it needs to be used. Dr. Dokania performs AV fistula creation as a day-care procedure under local anaesthesia.
Kidney transplant is the best treatment for eligible ESRD patients but not everyone qualifies. Contraindications include: active cancer, uncontrolled infection, severe heart disease not amenable to correction, morbid obesity, and active substance abuse. Dr. Dokania conducts comprehensive pre-transplant evaluation to assess eligibility.
