Prostate Enlargement (BPH) — Overview

Benign Prostatic Hyperplasia (BPH) — non-cancerous prostate enlargement — affects 50% of men by age 60 and 90% by age 85. It obstructs the urethra, causing poor urine flow, frequent urination (especially at night), urgency, straining and incomplete bladder emptying. Untreated, it leads to bladder damage, recurrent infections and kidney damage.

Frequently Asked Questions

Both treat prostate enlargement (BPH) without any external incision — everything is done through the urethra. TURP removes obstructing prostate tissue using electrical energy and is suitable for most prostate sizes with a 2–3 day stay. HoLEP uses a Holmium laser to enucleate the entire gland — bloodless, preferred for large prostates (>80 gm), with catheter removal next day and 1–2 day stay. HoLEP has the lowest long-term recurrence rate.
TURP and HoLEP commonly cause retrograde ejaculation (dry orgasm — semen goes backward into the bladder rather than out). This does not affect sexual pleasure or orgasm quality. Erectile function is generally preserved. For prostate cancer surgery (radical prostatectomy), nerve-sparing technique is used to maximise erectile function preservation.
Most patients return to desk work within 5–7 days after TURP or 3–5 days after HoLEP. The catheter is typically removed within 24–72 hours. Strenuous activity should be avoided for 4 weeks. Symptom improvement is immediate after catheter removal.
Yes — mild to moderate BPH is initially treated with medications: alpha-blockers (tamsulosin, alfuzosin) relax the bladder neck and 5-alpha reductase inhibitors (finasteride) shrink the prostate. Surgery is recommended when medications fail, urinary retention occurs, recurrent infections develop, or kidney function is affected.
