Bladder Cancer — Key Facts

  • Most common sign: painless visible blood in urine — never ignore, even once
  • 75% of cases are non-muscle-invasive at diagnosis — highly treatable
  • Risk factors: smoking (most significant), occupational chemical exposure, chronic UTI, cyclophosphamide
  • More common in men; peak age 60–70 years
  • Early-stage (Stage 0–I) has 5-year survival >90%

Diagnosis

  • Cystoscopy + biopsy — gold standard; direct visualisation of bladder lining
  • Urine cytology — detects cancer cells in urine
  • CT Urogram / MRI — staging and detection of upper tract tumours
  • Chest X-ray / CT chest — distant staging

Frequently Asked Questions

Blood in urine (haematuria) is never normal and always requires investigation. It can be caused by kidney stones (most common), urinary tract infection, bladder or kidney cancer, or kidney disease. Painless blood in urine in a person over 40 has a 10–15% risk of being due to bladder or kidney cancer and must be investigated with cystoscopy and CT urogram urgently.
Bladder cancer arises from the inner lining of the bladder (urothelium) — the most common symptom is blood in urine, treated with TURBT. Kidney cancer (renal cell carcinoma) arises from kidney tissue — often found incidentally on imaging, treated with nephrectomy. Both can present with haematuria; a CT urogram and cystoscopy distinguishes between them.
Non-muscle-invasive bladder cancer (75% of cases) rarely spreads and is curable with TURBT + BCG. Muscle-invasive cancer (25%) has a higher risk of lymph node and distant spread (lungs, liver, bone). Staging CT scan determines extent before deciding treatment. After radical cystectomy, 5-year survival is 50–65% for muscle-invasive disease.
