What is PCNL?

PCNL (Percutaneous Nephrolithotomy) is the gold-standard minimally invasive surgery for large kidney stones (>2 cm), staghorn calculi, and complex stones not treatable by ESWL or URSL. A small 1 cm puncture is made in the back under X-ray/ultrasound guidance; a channel is created into the kidney and the stone is fragmented using Holmium laser and removed.

Dr. Kanishk Dokania performs laser-assisted PCNL at Charak Hospital, Lucknow with a stone-free rate exceeding 85% and a hospital stay of 2–3 days.

When is PCNL Needed?

  • Kidney stones larger than 2 cm
  • Staghorn (coral) calculi filling the renal pelvis
  • Stones in horseshoe or anomalous kidneys
  • Stones that failed ESWL (shockwave therapy)
  • Multiple large stones in a single kidney
  • Stones causing obstruction, infection or bleeding

Frequently Asked Questions

Kidney stones larger than 2 cm are typically treated with PCNL. Smaller stones (5–20 mm) in the ureter can be treated with URSL (ureteroscopic laser). Stones 1–2 cm in the kidney can sometimes be treated with RIRS (flexible ureteroscopy) or ESWL if the position is favourable.
PCNL typically takes 60–90 minutes under general or spinal anaesthesia. A nephrostomy tube is placed at the end of surgery and removed after 24–48 hours once urine output is clear. Most patients are discharged in 2–3 days.
Yes — without preventive measures, stones recur in 50% of patients within 5 years. Prevention includes: drinking 2.5–3 litres of water daily, stone analysis to guide diet, 24-hour urine metabolic evaluation, reducing salt and animal protein, and regular ultrasound follow-up every 6–12 months.
PCNL is performed under anaesthesia so there is no pain during surgery. Post-operatively, there may be mild to moderate back discomfort where the puncture was made, typically controlled with oral pain medication for 3–5 days. Most patients are comfortable and mobile by Day 2.
