Abdominoperineal Resection (APR) for Rectal Cancer, Lucknow

APR for low rectal cancer and anal canal cancer β€” when sphincter preservation is not possible
← Back to Dr. Ashok Kumar's Profile

What is Abdominoperineal Resection (APR)?

APR is a major surgery for cancers of the lower rectum and anal canal where the tumour directly invades the anal sphincter muscles, making sphincter-saving surgery oncologically unsafe. The operation removes the rectum, anal canal, and sphincter muscles through two incisions β€” one abdominal and one perineal β€” resulting in a permanent end colostomy.

When is APR Necessary?

  • Low rectal cancer within 1 cm of the sphincter where negative margins cannot be achieved with sphincter preservation
  • Cancer directly invading the external sphincter or levator ani muscles
  • Pre-existing poor sphincter function where functional outcome of sphincter-saving surgery would be unacceptable
  • Anal canal squamous cell carcinoma not responding to chemoradiation (salvage APR)

Life After APR β€” Permanent Colostomy

A permanent end colostomy (sigmoid colostomy) is created in the left lower abdomen. Modern stoma appliances are discreet, odour-proof and allow a full, active lifestyle. Charak Hospital provides dedicated stoma care nursing support to help patients adjust confidently. Most patients resume all normal activities including travel, exercise and work within 6–8 weeks.

Frequently Asked Questions

No. With modern neoadjuvant chemoradiation and advanced surgical techniques like intersphincteric resection, many previously APR-requiring tumours can now be managed with sphincter preservation. Dr. Ashok Kumar carefully evaluates each case with MRI staging and discusses all options before recommending APR.
A colostomy uses the colon; an ileostomy uses the small intestine. After APR, a permanent sigmoid colostomy is created. Output is semi-formed, similar to normal stool. An ileostomy has more liquid output. The stoma site is carefully selected before surgery to ensure the best position for the appliance.
Yes. Most patients resume full physical activity including swimming, gym, sports and travel with modern stoma appliances. Stoma support groups and specialist stoma nurses at Charak Hospital provide guidance on all aspects of stoma management.
