When is Surgery Needed for Crohn's Disease?

Unlike ulcerative colitis, Crohn's disease cannot be cured surgically as it can affect any part of the GI tract. Surgery is indicated when medical therapy fails, complications arise, or for specific structural problems:

  • Bowel obstruction from strictures (narrowing)
  • Abscesses and fistulas not responding to medical treatment
  • Free perforation or severe haemorrhage (emergencies)
  • Failure of medical therapy (biologics, immunosuppressants)
  • Dysplasia or cancer in a Crohn's-affected segment

Surgical Options

  • Stricturoplasty β€” widening a narrowed bowel segment without resection; preserves bowel length. Preferred for short fibrotic strictures in the small intestine.
  • Segmental Resection β€” removal of the diseased bowel segment with primary anastomosis; used for longer strictures or fistulising disease.
  • Ileocaecal Resection β€” most common Crohn's surgery; removal of terminal ileum and caecum for ileocaecal Crohn's disease.
  • Fistula Surgery β€” drainage of abscesses, seton placement for complex perianal fistulas, and fistulotomy for low simple fistulas.
  • Proctocolectomy with Ileostomy β€” for severe colonic or perianal Crohn's disease that cannot be controlled; IPAA is generally not performed in Crohn's.

Perianal Crohn's Disease

Perianal fistulas, abscesses and skin tags are common in Crohn's disease. Management requires a combined medical (anti-TNF biologics) and surgical approach. Dr. Ashok Kumar's expertise in colorectal and perianal surgery enables comprehensive management of this challenging condition at Charak Hospital.

Frequently Asked Questions

Unlike UC, Crohn's disease cannot be cured surgically as it can recur anywhere in the GI tract. However, surgery dramatically improves quality of life by treating complications. Approximately 70-80% of Crohn's patients will require surgery at some point in their lifetime.
Stricturoplasty widens a narrowed bowel segment without removing it, preserving intestinal length β€” crucial in patients with multiple strictures who are at risk of short bowel syndrome. It is preferred for short fibrotic strictures in the small intestine where the disease is burnt-out (no active inflammation).
Complex perianal fistulas in Crohn's are treated with a combination of antibiotics, anti-TNF biologics (infliximab/adalimumab), and surgical drainage with seton placement. A seton is a thread passed through the fistula tract to allow drainage while protecting the sphincter. Definitive repair is attempted only when disease activity is controlled.
