What is a Hemicolectomy?

A hemicolectomy removes part of the colon along with its blood supply and regional lymph nodes. The remaining colon is joined back together (anastomosis). It is the standard curative surgery for cancers of the colon.

Types of Hemicolectomy

  • Right Hemicolectomy β€” removes caecum, ascending colon and hepatic flexure; for cancers of the right colon. The ileum is joined to the transverse colon.
  • Extended Right Hemicolectomy β€” also removes transverse colon; for hepatic flexure and transverse colon cancers.
  • Left Hemicolectomy β€” removes descending colon and splenic flexure; for left colon cancers. The transverse colon is joined to the sigmoid.
  • Sigmoid Colectomy β€” removes sigmoid colon; for sigmoid cancers or severe diverticular disease.
  • Total Colectomy β€” removes entire colon; for familial adenomatous polyposis (FAP) or synchronous cancers.

Laparoscopic vs Open Hemicolectomy

Dr. Ashok Kumar performs both laparoscopic and open hemicolectomy. Laparoscopic colectomy is preferred for eligible patients β€” shorter hospital stay (3–4 days vs 5–7 days), faster recovery, less pain and equivalent cancer outcomes. Open surgery is used for advanced tumours with adjacent organ involvement.

Frequently Asked Questions

A minimum of 12 regional lymph nodes must be removed and examined for accurate cancer staging. Dr. Ashok Kumar follows the principle of complete mesocolic excision (CME) with central vascular ligation (CVL) to maximise lymph node harvest and reduce local recurrence.
In most planned hemicolectomies, the bowel ends are joined (anastomosis) and no stoma is needed. A temporary stoma may occasionally be created to protect a low anastomosis or in emergency surgery. Permanent colostomy is rarely needed for colon cancer treated with hemicolectomy.
Laparoscopic hemicolectomy: 3–4 days. Open hemicolectomy: 5–7 days. Return to light activity in 2–3 weeks; full recovery in 4–6 weeks. Enhanced Recovery After Surgery (ERAS) protocols are followed at Charak Hospital.
