What is Obstructed Defecation Syndrome?

ODS is a functional disorder where patients have difficulty evacuating the rectum despite straining β€” the stool is present but cannot be expelled. It is characterised by: excessive straining, sensation of incomplete emptying, need for digital assistance (finger assistance to pass stool), and multiple failed attempts. ODS is caused by structural abnormalities: rectocoele (anterior bulge of rectal wall), internal rectal intussusception, or enterocele.

Diagnosis

  • Defaecating Proctogram β€” dynamic X-ray study while the patient attempts defecation; shows rectocoele, intussusception, enterocele
  • MRI Defaecography β€” superior imaging without radiation
  • Anorectal Manometry β€” rules out anismus (paradoxical sphincter contraction)
  • Colonoscopy β€” to exclude structural lesions

Treatment

  • Conservative first β€” high-fibre diet, laxatives, biofeedback for anismus, pelvic floor physiotherapy
  • STARR Procedure β€” Stapled Transanal Rectal Resection; removes excess rectal tissue causing obstruction using circular staplers passed through the anus. Day-care procedure.
  • Laparoscopic Ventral Rectopexy (LVR) β€” fixes the rectal wall to the sacrum with a mesh, correcting rectocoele and intussusception simultaneously. Preferred for complex ODS with combined prolapse.

Frequently Asked Questions

The STARR procedure achieves significant improvement in ODS symptoms in 70–80% of patients. Constipation scores, straining and sensation of incomplete emptying all improve. Some patients experience temporary urgency after STARR which usually settles within 3–6 months.
ODS is a specific type of constipation where the problem is in emptying (outlet obstruction) rather than colonic transit. Standard laxatives often don't help ODS significantly. Biofeedback for anismus and surgical correction of structural abnormalities are the key treatments.
Yes. Internal rectal intussusception (a type of incomplete rectal prolapse) is one of the main causes of ODS. Laparoscopic ventral rectopexy addresses both conditions simultaneously by fixing the rectum to the sacrum and repairing the rectocoele.
