What is Sacral Nerve Stimulation?

Sacral Nerve Stimulation (SNS) — also known as sacral neuromodulation or InterStim therapy — is a minimally invasive implantable device that delivers mild electrical impulses to the sacral nerves, which control the bowel, bladder and pelvic floor. It modulates the neural pathways to improve bowel control and reduce unwanted symptoms.

Conditions Treated

  • Fecal Incontinence — the most common indication; most effective for neurogenic incontinence and post-sphincteroplasty failures
  • Obstructed Defecation Syndrome (ODS) — reduces straining and sensation of incomplete emptying
  • Urinary Incontinence / Overactive Bladder — in coordination with urology
  • Chronic Constipation — slow transit constipation not responding to medical treatment

The SNS Procedure

  • Stage 1 — Test Stimulation (Peripheral Nerve Evaluation): A temporary lead is inserted near the S3 sacral nerve under local anaesthesia (day-care). Worn for 2–3 weeks while the patient records bowel diary. If >50% improvement, permanent implant is offered.
  • Stage 2 — Permanent Implant: The permanent generator (similar to a pacemaker) is implanted under the skin of the buttock under local anaesthesia. Typically day-care or overnight stay.
  • Battery life: 3–7 years depending on settings

Frequently Asked Questions

SNS achieves a 50%+ reduction in incontinence episodes in 70–80% of patients. Complete continence is restored in approximately 40–50% of cases at 5-year follow-up. It is effective even in patients who have failed sphincteroplasty.
Yes. The device can be adjusted, reprogrammed or removed at any time. If the patient does not respond or develops complications, the device is simply explanted. The test phase before permanent implantation allows patients to assess response risk-free.
Newer SNS devices (e.g., InterStim II, InterStim Micro) are MRI-conditional — certain MRI scans can be performed with the device in a specific mode. Consult Dr. Ashok Kumar and the radiologist before scheduling any MRI after implantation.
