What is Stapled Haemorrhoidopexy?

Stapled Haemorrhoidopexy β€” also called MIPH (Minimally Invasive Procedure for Haemorrhoids) or PPH (Procedure for Prolapse and Haemorrhoids) β€” is a modern surgical technique for Grade 3 and 4 haemorrhoids. Using a circular stapling device inserted through the anus, a ring of prolapsed haemorrhoidal tissue is removed and the remaining tissue is stapled back into its anatomically correct position inside the anal canal.

Advantages Over Conventional Haemorrhoidectomy

  • No external wound β€” the entire procedure is inside the anal canal, above the pain-sensitive area
  • Significantly less pain β€” most patients need only mild analgesics for 1–2 days
  • Day-care procedure β€” discharged same day or next morning
  • Rapid recovery β€” return to work in 2–3 days (vs 2–3 weeks for conventional surgery)
  • Shorter operation time β€” typically 20–30 minutes

Which Grades of Haemorrhoids are Suitable?

  • Grade 3 β€” prolapse requiring manual reduction; ideal candidate for MIPH
  • Grade 4 β€” permanently prolapsed; can be treated with MIPH or conventional haemorrhoidectomy depending on size
  • Grade 1 & 2 β€” do not usually need surgery; managed with dietary changes and rubber band ligation

Frequently Asked Questions

MIPH has a recurrence rate of approximately 5–10% at 5 years β€” slightly higher than conventional haemorrhoidectomy. Recurrence is more common if the staple line is not placed at the correct level. Maintaining a high-fibre diet, adequate hydration and avoiding straining significantly reduces recurrence risk.
MIPH is not suitable for: large external haemorrhoids with skin tags (these need conventional excision), thrombosed haemorrhoids requiring emergency surgery, or anal fissure/fistula present simultaneously. Dr. Ashok Kumar will examine you and recommend the most appropriate treatment.
Yes β€” MIPH is performed under spinal or general anaesthesia as a day-care procedure. It is not performed under local anaesthesia (unlike rubber band ligation for Grade 1–2 haemorrhoids). Most patients go home 4–6 hours after the procedure.
