What is the Whipple Procedure?

The Pancreaticoduodenectomy (Whipple procedure) is a major abdominal surgery that removes the head of the pancreas, the duodenum (first part of small intestine), gallbladder, lower bile duct, and sometimes part of the stomach. The remaining digestive organs are reconnected to allow normal digestion. It is the only curative surgical option for cancers of the head of the pancreas, ampulla of Vater, distal bile duct and duodenum.

When is Whipple Surgery Performed?

  • Cancer of the head of the pancreas
  • Ampullary carcinoma (cancer at the junction of bile duct and duodenum)
  • Carcinoma of the distal common bile duct
  • Duodenal cancer
  • Chronic pancreatitis with head mass (Frey's/Beger procedure variants)
  • Cystic tumours of the pancreatic head (IPMN, mucinous cystic neoplasm)

Frequently Asked Questions

In experienced hands at a high-volume centre, the operative mortality of Whipple surgery is below 3–5%. Major complications include delayed gastric emptying (20%), pancreatic fistula (15%) and bile leak (5%). Dr. Ashok Kumar's extensive experience at SGPGI and Charak Hospital ensures optimal outcomes.
Laparoscopic (minimally invasive) Whipple is technically feasible and performed at high-volume centres. In selected patients it offers faster recovery. Dr. Ashok Kumar discusses the approach (open vs laparoscopic) based on tumour characteristics, patient fitness and technical feasibility.
The pancreas continues to produce digestive enzymes through the remaining pancreatic tissue, which is reconnected to the small bowel. Many patients need pancreatic enzyme replacement therapy (PERT) tablets with meals. Diet is gradually upgraded from liquids to solids over 4–6 weeks. Most patients maintain good nutritional status with appropriate supplementation.
