What is Undescended Testis (Cryptorchidism)?

Undescended testis (cryptorchidism) is a condition where one or both testes fail to descend into the scrotum during foetal development. It occurs in 3–5% of term newborns and 30% of premature boys. About 70% descend spontaneously by 3 months of age; those remaining undescended by 6 months require surgical treatment.

⚠️ Orchidopexy should be performed ideally between 6–18 months of age. Delaying beyond 2 years significantly increases the risk of infertility and testicular cancer.

Why Early Treatment Matters

  • Fertility protection — delay damages sperm-producing cells irreversibly after age 2
  • Cancer risk — undescended testis has 3–5× higher risk of testicular cancer
  • Testicular torsion — ectopic testis is at higher risk of twisting (emergency)
  • Psychological — empty scrotum causes distress; early correction prevents this

Orchidopexy Surgery

Orchidopexy (fixation of undescended testis in the scrotum) is performed under general anaesthesia through a small groin incision. The testis is mobilised from its ectopic position and fixed into the scrotum. Day-care or overnight procedure. Recovery: 3–5 days; return to school in 1 week.

For impalpable testes (not felt in groin), laparoscopy is performed first to locate and then stage the orchidopexy.

Frequently Asked Questions

Yes — approximately 70–80% of undescended testes in term newborns descend spontaneously within the first 3 months of life. However, testes that have not descended by 6 months are unlikely to descend further and require hormonal evaluation or surgical orchidopexy by 12–18 months.
Early orchidopexy (before 18 months) significantly improves fertility outcomes. Unilateral cryptorchidism treated early has fertility rates close to normal. Bilateral untreated cryptorchidism leads to infertility in up to 75% of men. The earlier the surgery, the better the sperm-producing cell preservation.
Yes — orchidopexy requires general anaesthesia in children. It is a safe, well-established procedure performed as day-care or overnight surgery. Most children are active and comfortable within 24–48 hours. The groin incision is small (2–3 cm) and heals well.
