What is Hydronephrosis?

Hydronephrosis is swelling (dilation) of the kidney due to urine collecting inside it β€” caused by obstruction or backflow. The most common cause requiring surgery in adults and children is UPJO (Ureteropelvic Junction Obstruction) β€” a blockage where the kidney pelvis meets the ureter.

Symptoms

  • Flank pain β€” especially after drinking large volumes (Dietl's crisis)
  • Recurrent urinary tract infections
  • Kidney stones (stagnant urine promotes stone formation)
  • Hypertension
  • Incidental finding on ultrasound or CT (often asymptomatic)

Treatment β€” Pyeloplasty

Laparoscopic Pyeloplasty

  • Gold standard for UPJO β€” excises the blocked segment, reconstructs the kidney-ureter junction
  • Success rate: >95% (equivalent to open surgery)
  • Performed laparoscopically β€” 3 small keyhole incisions
  • Hospital stay: 2–3 days; return to work: 1–2 weeks
  • DJ stent kept for 4–6 weeks post-operatively
  • Follow-up renogram (DTPA scan) at 3 months confirms drainage

Frequently Asked Questions

UPJO is diagnosed with: Ultrasound KUB (shows hydronephrosis), DTPA/MAG3 renogram (confirms obstruction and measures differential kidney function), CT urogram (defines anatomy, rules out crossing vessel), and whitaker test for equivocal cases. Obstruction causing progressive kidney function loss (<40% differential function) is the primary indication for surgery.
Indications for pyeloplasty include: symptomatic UPJO (pain, recurrent UTI, stones), progressive hydronephrosis on serial imaging, differential renal function <40% (poor kidney contribution), and split function declining on sequential scans. Mild asymptomatic hydronephrosis with well-preserved function can be monitored.
No. Laparoscopic dismembered pyeloplasty (Anderson-Hynes technique) achieves equivalent outcomes to open surgery (>95% success) with significantly smaller incisions, less pain, shorter hospital stay (2–3 days vs 5–7 days) and faster recovery (1–2 weeks vs 4–6 weeks). Dr. Dokania performs laparoscopic pyeloplasty at Charak Hospital.
