What is Hypospadias?

Hypospadias is a common congenital birth defect in boys where the urethral opening (meatus) is located on the underside of the penis rather than at the tip. It occurs in approximately 1 in 300 male births. Associated findings: downward curvature of the penis (chordee), abnormal foreskin (dorsal hood), and sometimes undescended testis.

Surgical Repair

When & How

  • Ideal age for repair: 6–18 months (before toilet training and preschool)
  • Distal (mild) hypospadias — MAGPI or TIP (Snodgrass) urethroplasty; single-stage
  • Proximal (severe) hypospadias + chordee — staged repair; first stage: chordee correction; second stage: urethroplasty (6 months later)
  • Anaesthetic: general anaesthesia; day-care or overnight stay
  • Urethral catheter for 5–10 days post-operatively

Frequently Asked Questions

Yes — hypospadias can be repaired at any age, though results are best when performed in infancy (6–18 months). Adults with untreated or failed childhood hypospadias can undergo complex reoperative urethroplasty. However, adult repair is technically more challenging with higher complication rates than primary childhood repair.
The most common complication is urethrocutaneous fistula (a small hole in the urethra repair site causing urine leakage — occurs in 5–15%) and urethral stricture or meatal stenosis. Minor fistulas may close spontaneously; larger ones require reoperation after 6 months. Overall, 85–90% of hypospadias repairs give excellent long-term outcomes.
Most men with distal hypospadias have normal fertility. Proximal hypospadias (if involving the urethra near the scrotum) may affect ejaculation and sperm delivery if untreated or if repair was not adequate. Testosterone levels are generally normal. Hypospadias associated with undescended testis may have a mild impact on fertility.
