What is Overactive Bladder?

Overactive Bladder (OAB) is a condition characterised by a sudden, strong urge to urinate that is difficult to control — with or without urge urinary incontinence (leakage). It severely impacts quality of life, sleep, work and social activities. OAB affects 15–20% of adults; many don't seek help due to embarrassment.

OAB is caused by abnormal involuntary contractions of the bladder muscle (detrusor overactivity) triggered before the bladder is full.

Symptoms (OAB Wet vs Dry)

  • Urgency — sudden compelling urge to urinate, difficult to defer
  • Frequency — urinating more than 8 times per day
  • Nocturia — waking more than once at night to urinate
  • Urge incontinence (OAB Wet) — leakage associated with urgency

Step-Up Treatment

  • Conservative — Bladder training, fluid management, reduce caffeine/alcohol, timed voiding schedule
  • Pelvic Floor Physiotherapy — for urge suppression techniques
  • Medications — Anticholinergics (solifenacin, oxybutynin) or beta-3 agonist (mirabegron)
  • Bladder Botox Injection — 100 units into bladder wall; lasts 6–9 months; 60–70% success
  • Sacral Nerve Stimulation (SNS) — implantable device for refractory OAB; 70–80% success

Frequently Asked Questions

OAB refers to the urgency, frequency and nocturia syndrome — which may or may not include urinary leakage. When OAB includes involuntary urine leakage with urgency, it is called urge urinary incontinence (OAB wet). Stress incontinence (leakage on coughing/exercise) is a different condition caused by sphincter weakness, not detrusor overactivity.
Anticholinergics and beta-3 agonists typically show improvement within 4–8 weeks. If one medication doesn't work, switching to another class (e.g., from anticholinergic to mirabegron) may be effective. Maximum benefit is assessed at 4–12 weeks. Many patients need combination therapy.
Botox (onabotulinumtoxinA 100 units) is injected into the bladder muscle at 20–30 sites through a cystoscope under local anaesthesia. It relaxes the overactive bladder muscle, reducing urgency and leakage. The procedure takes 15–20 minutes and effects last 6–9 months, after which repeat injection is required. Success rate: 60–70%.
