Urethral Stricture — Overview

A urethral stricture is a narrowing of the urine passage (urethra) caused by scar tissue from trauma, infection, catheterisation, or previous surgery. Symptoms: weak or split urine stream, straining, dribbling, incomplete emptying, recurrent UTI, and in severe cases — complete urinary retention.

Treatment — Urethroplasty

Surgical Options

  • Optical Internal Urethrotomy (OIU) — endoscopic incision for short strictures. Day-care. Higher recurrence (40–60% at 5 years).
  • Buccal Mucosal Graft Urethroplasty — gold standard for long or recurrent strictures. Graft from inner cheek reconstructs the urethra. Success rate: 85–95% at 5 years. Hospital stay: 3–5 days.
  • Anastomotic Urethroplasty — excision and end-to-end repair for short bulbar strictures. Highest success rate (>95%).

Vesicovaginal Fistula (VVF) Repair

A VVF is an abnormal connection between the bladder and vagina causing continuous urinary leakage through the vagina. Causes: obstetric injury, gynaecological surgery, radiation. Dr. Dokania performs transvaginal and transabdominal repair with tissue interposition. Success rate: 85–95% with primary repair.

Frequently Asked Questions

Hospital stay: 3–5 days. A urethral catheter is kept for 3–4 weeks while the graft heals. Return to desk work: 3–4 weeks. Full activity: 6 weeks. Most patients experience dramatically improved urine flow immediately after catheter removal. Follow-up urine flow rate and urethroscopy at 3 months confirm success.
Internal urethrotomy (OIU) has a recurrence rate of 40–60% within 5 years — especially for long or penile strictures. Buccal mucosal graft urethroplasty has a long-term success rate of 85–95% at 5 years. For patients with multiple previous failed treatments, urethroplasty is the preferred definitive option.
Common causes: trauma (straddle injury, pelvic fracture), previous urethral catheterisation, gonorrhoeal urethritis, lichen sclerosus (skin condition affecting urethra), and previous anorectal/prostate surgery. Many cases are idiopathic (no identifiable cause). Retrograde urethrogram and cystoscopy confirm the diagnosis and measure stricture length.
